All campers (youth/adults) must provide a completed physical fitness form at registration. Please use the one provided. ​

Companies can also generate​ Adventure Application Forms (AAF's) through Scout Tracker.  AAF's must be signed off by your GC or SRM before uploading the documentation to the drop box as a PDF.  
​Upon completing any additional forms, please save the file as a PDF and set the file name with the following format:
Example: For the group 1st RubberBoot the file name for the Attendance Form would be: GEN23_1stRubberBoot_attendanceform.pdf
